You may qualify for $400
You are required to complete the Surgery Decision Support® (SDS) program if your doctor recommends elective lower back surgery, hip or knee replacement, weight loss surgery, or hysterectomy. A $400 penalty will apply if you do not complete the SDS program at least 30 days before your scheduled surgery. You can avoid the penalty and qualify for a $400 prepaid card by completing the SDS program and learning about your options.
